The US Department of Labor’s Occupational Safety and Health Administration (OSHA) cited a Pennsylvania facility with multiple safety violations after a six-month investigation, according to The New York Times. OSHA inspected a facility in Palmyra owned by Exel, a company that packages Hersey chocolates. OSHA launched the investigation after employees alleged Exel had covered up worker injuries. The employees who complained to OSHA were student workers part of an international exchange program. OSHA discovered that Exel failed to report 42 serious injuries in a four year span—43% of all injuries. Many of the injuries required medical treatment. The US Department of Labor’s Occupational Safety and Health Administration (OSHA) cited a Jeffersonville, Indiana facility after a worker was killed in August of 2011, according to the Courrier-Journal.com. The citations issued following the recent inspecting bring the total amount of OSHA penalties against Jeffboat, an Indiana ship building company, to 46 since May of 2010. 54-year-old Steven Duncan died at the facility in August when a transfer car he was working underneath activated, raising him on its hydraulic jack and crushing him against a barge. OSHA issued Jeffboat nine violations, including failure to implement lockout procedures, failure to train employees to operate and work safely around transfer cars, and failure to ensure electrical outlets are deactivated before employees work on them. “Jeffboat has a responsibility to ensure that its employees are properly protected from known workplace hazards,” said Indianapolis Regional Director Ken Gilbert. “Failing to ensure workers’ safety and…

The U.S. Department of Labor’s Occupational Safety and Health Administration (OSHA) has cited Publix Supermarkets in Florida with safety violations following a worker injury in which an employee lost his hand, according to Jacksonville.com. The accident occurred in September of 2011, when a worker severed his hand while cleaning a conveyor belt, according to reports. OSHA cited Publix with 16 safety violations, including the most serious categories; repeat violations and willful violations. The proposed fines total $182,000. OSHA placed Publix in its Severe Violator Enforcement Program. Publix facilities will be subject to more frequent and random inspections. A Publix employee in Deerfield Beach was crushed by a truck in 2007, and OSHA also cited the company for unsafe working conditions at a Georgia facility in 2008.
