The U.S. Department of Labor’s Occupational Safety and Health Administration (OSHA) issued 17 safety violations to a hotel in Hawaii for hazardous working conditions, according to Hawaii News Now. OSHA officials inspected a hotel in Waikiki owner by the Halekulani Corp. Fourteen of the 17 violations were classified as “serious”. Citations were issued for employers failing to provide proper personal protective equipment to workers, a lack of fire extinguishers and a lack of appropriate electrical equipment. Proposed penalties total $49,000. “The hotel industry is one of the biggest employers in Hawaii, and it’s critical that owners and operators have strong safety and health programs,” said regional OSHA director Galen Lemke. “Halekulani has an obligation to protect its staff and ensure a safe working environment.” Halekulani will have 15 business days after receiving its official statement from OSHA to agree to pay the fines or request a hearing.
The U.S. Department of Labor’s Occupational Safety and Health Administration (OSHA) announced it would increase its efforts in monitoring the safety and working conditions of temporary workers contracted to companies through staffing agencies. The Department of Labor sent out a memo to its regional offices on April 29, covering some things that inspectors need to look for specifically at facilities utilizing temporary workers. “Recent inspections have indicated problems where temporary workers have not been trained and were not protected from serious workplace hazards due to lack of personal protective equipment when working with hazardous chemicals and a lack of lockout/tagout protections, among others,” the agency’s memorandum reads. A few weeks ago, we brought you the story of a temp worker killed in a Chicago facility after he was burned by hazardous chemicals. According to OSHA Chief David Michaels, OSHA decided to launch its initiative after reviewing statistics that revealed many...

Last week, we brought the story of a barge explosion on a river in Mobile, Alabama, in which three workers were severely burned. One of the workers, Casey Tyson, is listed in fair condition and is expected to be released from the University of South Alabama Medical Center, but Justin Benoit and George Erickson were both listed in critical condition at the same hospital, according to Alabama Online. The company that was responsible for the explosion, Oil Recovery Co., has had issues in the past with the U.S. Department of Labor’s Occupational Safety and Health Administration (OSHA). Oil Recovery Co. was fined $1,000 by OSHA in 1991 for several safety violations. During the inspection that resulted in that fine, OSHA determined that Oil Recovery Co. failed to provide workers with personal protective equipment and failed to have designated employees with knowledge of OSHA rules and regulations. OSHA also cited the...

Federal officials have cited Ball Aerosol and Specialty Container in Hubbard, Ohio, with 11 safety violations, according to Workers Compensation. Officials from the U.S. Department of Labor’s Occupational Safety and Health Administration (OSHA) inspected the facility in October 2012, after receiving a complaint about unguarded machines. OSHA cited violations for a lack of guards on moving equipments and machines, various fire extinguisher violations, failure to guard blades and a lack of guards on rotating parts. Proposed fines total $589,000. “Ball Aerosol’s management made a decision to continue to expose machine operators to serious amputation hazards,” said OSHA’s Nick Walters. “Workers should not be asked to take such risks, and OSHA will not tolerate such disregard for worker safety.” The violations relating to fire extinguishers—three in total—were classified as repeat violations. Ball was cited for similar ones in 2009. Ball Aerosol was placed in OSHA’s Severe Violator Enforcement Program. It will...

A tragic workplace accident claimed the life of a Southern California Edison worker on April 25, according to ABC7. The worker, identified as 38-year-old Jose Raul Ros, was working with underground equipment in Menifee, California when an explosion killed him and injured three others. Ros was performing maintenance work in an underground vault that powers a nearby facility. “The boom sounded like maybe somebody was in an accident, or somebody ran into something, that was my first thought,” said witness Dave Arnold, who lives near the site. “It’s sad, these are utility workers, and they have a job to do, but if it’s a malfunction, or human error, you just don’t know.” It will not be long before the US Department of Labor’s Occupational Safety and Health Administration (OSHA) sends officials to inspect the worksite. If Southern California Edison was in violation of any federal safety regulations, it could be...

According to Fox News, federal officials arrived in Mobile, Alabama to examine the site of a barge explosion on a river that left three workers with critical burns on April 25. A crew on a nearby ship evacuated following the explosion, according to the U.S. Coast Guard. Officials from the U.S. Department of Labor’s Occupational Safety and Health Administration (OSHA) arrived in Alabama to inspect the site Oil Recovery Co. site where the explosion occurred. The investigation remains in the early stages and not much information is available at this point. “They are still working on verifying the name of the company,” said Mark D’Aquino, an OSHA director in Atlanta. The Coast Guard announced it would be conducting its own investigations as well. “It literally sounded like bombs going off around,” said witness Alan Waugh, who lives in downtown Mobile. The sky just lit up in orange and red.
The US Department of Labor’s Occupational Safety and Health Administration (OSHA) has cited a facility in North Jackson, Ohio, with 8 safety violations, according to WKYC-TV. OSHA officials inspected the Extrudex Aluminum facility following a fatal accident a little over a year ago. John Tomlin Jr., 21, died last year when he was crushed to death underneath a cart of hot aluminum parts. OSHA officials determined that management acted with disregard or indifference to safety regulations that resulted in the accident. The report alleges that Extrudex Aluminim “could have prevented this tragic loss by protecting workers from hazards unique to its operation.” “Workers should not be asked to take such risks, and OSHA will not tolerate such disregard for human safety,” said OSHA administrator Howard Eberts. This inspection resulted in a $175,000 fine and places Extrudex in OSHA’s severe violators program. It will be subject to more OSHA inspections in...

The U.S. Labor Department claims that Metra—the Illinois Regional Transportation Authority’s commuter rail division—unlawfully retaliated against a whistleblower, according to the Chicago Tribune. The whistleblower, identified as a 22-year-old male employee working for Metra as a signalman, filed a safety complaint with the department’s Occupational Safety and Health Administration (OSHA). After filing, he saw his hours decrease until his position was eventually eliminated, according to his complaint. The employee initially complained about not having enough time to perform standard safety tests on signals and requested overtime hours to complete the task, which was denied. “An employer does not have the right to retaliate against employees who report safety issues,” Nick Walters, an OSHA regional administrator in Chicago said. “When employees can’t report safety concerns on the job without fear of retaliation, worker safety, and in this case, passenger safety on Metra, [it] becomes a serious concern.” OSHA ordered Metra to...

Orange Mayor Dwayne Warren has ordered the Orange Public Library in New Jersey to be temporarily closed until safety issues are corrected, according to NJ Online. The library had previous issues related to lead paint and asbestos that lead to a two-month closure in 2010. Officials from the US Department of Labor’s Occupational Safety and Health Administration (OSHA) and the state Division of Public Safety inspected the facility again and cited violations due to “extensive structural issues.” The inspection came after Orange council president Teancy Eason fell in the library and alerted the state officials. She fell on a staircase due to a missing rail—which is an OSHA violation. OSHA has not yet released a summary of its findings. “It was a threat and a safety issue to patrons and workers there,” said Warren. The mayor said estimated repair costs total over $1 million. It is good to see Mayor...

An explosion at a fertilizer plant in West, Texas on April 17 caught the attention of the nation—as is evident in this terrifying footage caught by a witness. At least 14 people died in the blast and over 200 more were injured, according to the Associated Press. The fertilizer plant initially caught fire before a larger explosion, which killed many firefighters that were attempting to extinguish the blaze. Fertilizer plants contain methane and other extremely combustible materials. According to Reuters, the plant was storing over 270 tons of ammonium nitrate fertilizer at the time of the fire. Federal law requires high amounts to be registered with the Department of Homeland Security (DHS), which it was not. The Texas Department of State Health Services claimed it was aware of the amount of the chemical being stored, but did not report it to DHS. Even more disturbing are the reports that the...
